Please fill out and submit the form below. Based on your child's age and challenges, I will send you some questions and prompts so I can better understand your child's needs. I will create your home program and we'll schedule a virtual meeting, usually about an hour, to teach you how to use it.
The cost for this is $200 USD. If your financial circumstances do not permit this, let me know on the form and we'll try and come to an arrangement.
